Validation of the Implementation Model of the Comprehensive Policy Document on Health System Care with an Approach to the Seventh Development Plan

Abstract

The purpose of this study was to design and validate a comprehensive model for implementing health system policies within the framework of Iran’s Seventh Development Plan, emphasizing service standardization, health data registration, legal frameworks, and equity in access to healthcare services. This quantitative study employed confirmatory factor analysis (CFA). Data were collected through a researcher-developed questionnaire covering four main dimensions: clinical guideline standardization, health data registration in the electronic health record (EHR) system, establishment of health-related rules and regulations, and insurance policies with financial incentives. A stratified sample of healthcare managers, experts, and professionals participated. Goodness-of-fit indices including χ², GFI, AGFI, RMSEA, and χ²/df were applied to assess model adequacy. The CFA results indicated that the proposed model demonstrated an acceptable fit (GFI=0.91, AGFI=0.90, RMSEA=0.048, χ²/df=2.31). All factor loadings exceeded 0.60, confirming convergent validity. Dimensions of medical data standardization, health data registration, establishment of legal frameworks, and insurance equity were statistically significant. The overall goodness-of-fit indices confirmed that the model sufficiently explained the operational dimensions of health policy implementation in Iran. The validated model highlighted that achieving the objectives of the Seventh Development Plan requires the simultaneous integration of technical (clinical guidelines and digital data systems), legal (rules and regulations), and socio-economic (insurance equity and financial incentives) dimensions. This framework can serve as a roadmap for Iranian policymakers to enhance coherence, efficiency, and equity in the national health system.
